When you come by the office with a completed Enrollment Form and Medical Release Form, You will also need to complete a registration card. Please be prepared to answer the following:

FRONT SIDE:
1. Child(ren)'s Last Name
2. Phone
3. Names of Children in Meadowood Kid's MDO | Age | Birthdate (including year)
4. Does child have seizures? | Instructions
5. List any allergies, foods child should not eat, or special needs
6. Mother's Name | Work Phone
7. Father's Name | Work Phone
8. Home Address
9. Person bringing child (if not parent) | Phone

REVERSE SIDE:
10. Persons other than parents to be contacted in case of illness or accident when parents cannot be reached.
11. Doctor
12. Doctor's Office location
13. Church family attends
14. Parent's signature and date after the following terms: "I have read the Meadowood Kid's MDO Policies and agree to abide by them. The Meadowood Kid's MDO Director has a signed and notarized Medical Release Form for my child(ren) on file.
